BUDAYA KESELEMATAN PASIEN DITINJAU DARI PENERAPAN 12 BENAR OBAT DAN MEDICATION ERROR: LITERATURE REVIEW

Abstract

Latar Belakang: Seringnya terjadi kejadian tidak diinginkan (KTD) di rumah sakit salah satunya ialah kecelakaan karena kesalahan pemberian obat yang disebabkan kurangnya ketelitian sehingga menimbulkan dampak keselamatan bagi pasien.

Tujuan: Untuk mengetahui budaya keselamatan pasien dari penerapan 12 benar obat dan Medication Error.

Metode: Sistematis tinjauan dilakukan pada beberapa basis data internasional yang diterbitkan dari tahun 2015 hingga 2020. Pengumpulan jurnal berbagai literature dengan menggunakan situs jurnal yang telah terakreditasi seperti Google Scholar, Pubmed, dan Biomed Central dengan menggunakan clinical key atau kata kunci “Patient Safetyâ€+ Medication Eror†dan “Keselamatan Pasienâ€+ Medication Erorâ€.

Hasil: Dari 10 artikel jurnal untuk prinsip 12 benar obat yang paling banyak mengalami kesalahan yaitu prinsip benar dosis >50%, dan sisanya kesalahan pada prinsip benar pasien, benar obat, benar rute sertabenar waktu & frekuensi. Dan untuk terjadinya Medication Error berada pada tahap fase administrasi dan prescribing error.

Kesimpulan: Jadi peneliti menyimpulkan bahwa kesalahan yang terjadi pada prinsip benar dosis dan medication error berada pada tahap Fase Administrasi.

Background: There are frequent unwanted events (KTD) in the hospital, one of which is accidents due to errors in administering drugs due to a lack of accuracy that has a safety impact on patients.
Objective: To determine the patient safety culture from the correct application of drugs and drugs
Medication Error.
Methods: Systematic reviews were carried out on several international databases published from 2015 to 2020. Journal collection of various literature using accredited journal sites such as Google Scholar, Pubmed, and Biomed Central using clinical keys or the keyword "Patient Safety" + Medication Error "and" Patient Safety "+ Medication Error".
Results: Of the 10 journal articles, for the 12 correct principles, the most error occurred, namely the correct dosage principle> 50%, and the rest were errors.
correct patient principle, correct medication, correct route and correct time & frequency. And the occurrence of Medication Error is at the administrative and prescribing error phase.
Conclusion: So the researcher concludes that the errors that occur in the principle of correct dosage and medication errors are at the Administration Phase.
